Forms Check List - Fall 2008
There are several forms that need parental attention. Please take time
with your son to get the forms filled out and returned to Coach or Mrs.
Gail at the Float Test Monday, 4 August 2008 at Konieczny’s pond*.
The check list below should help insure that each form that needs a parent/guardian
signature has, in fact, received that signature; that Uni-Suit sizes
have been identified; that addresses and phone numbers are complete**.

of Liability Form
___ USRA 2008 Waiver [Waiver
for all the regattas (except Head of the Charles) that SJJ crew
will attend. Coach will make copies to send to each regatta committee.]
___ Check for $460.00 made out to SJJ Crew. Include check with forms;
post date for
up to two weeks, if necessary; we will hold for deposit after the date on
the check.
If you are ordering a Uni-Suit ($85) and/or a Parka ($75), include the
appropriate sum of money in the check or write a separate check
Please be sure that all the forms are completely filled out** and returned
to Coach or Mrs. Gail at the FLOAT TEST.
Do the family a favor: Post the Regatta schedule (separate sheet of
paper) on your refrigerator, so everyone in the family knows what it
is.
As soon as all the forms are in we can assemble a team roster and distribute
it. That will make life a lot easier for all of us, especially those
who are coordinating lodging, transportation, and food.
*Konieczny’s pond is off Rte 795, ½ mile east of the intersection
of 795 and Fostoria Road. Turn right onto a dirt road that disappears
into the woods; wind thru the woods to a clearing. There’s the
house and the pond.
** "Completely filled out" is really tricky! It means full
address to the nearest zip code, weight to the nearest pound, telephone
number to the nearest area code. Coach is serious. This is the trickiest
part of the entire season!
