Health OfficeSt. John's Jesuit Medication GuidelinesSt. John's Jesuit High School follows the diocesan guidelines and Ohio School Law regarding medication administration. Medication will be administered by trained school personnel only when written permission is obtained from the physician and parent. A physician and parent medication administration form can be obtained from the health office or the St. John's web site. Medication should be brought to the health office an be given to the nurse or office personnel. Students may not carry medication during the school day. If your son carries an inhaler, please complete the asthma inhaler form and return to the health office. It is advised that an additional inhaler be located in the health office in the event that your son is unable to find his inhaler or appears in the office in distress. For students who have an order for an Epi-pen or who carry an Epi-pen, we request that a duplicate pen be left in the health office. In addition, an Allergy Action Plan form should be completed by a parent/physician. Medication must come to school in pharmacy-labeled bottles including: student's name, physician's name, drug, dose and time to be given. The pharmacy is able to divide the medication into separate labeled bottles for home and school. This will help alleviate the risk of missing an important dose. St. John's medication guidelines apply to both prescription and over-the-counter medicines. If your son requires an OTC medicine while in school, we request bottled medicine and completed parent medication form be brought to the health office after 9:00 am. Please do not bring medicine in a plastic bag. We strongly encourage students to take any OTC medication before and after school if possible. |
